Need to File an Injury or Illness Claim?

Count on us for guidance

If you need to submit an injury or illness (disability) claim, simply complete the form below. You can also visit any of our branch locations or give us a call at (888) 213-2848.

To qualify for coverage, you’ll need an active Injury & Illness policy on your American 1 loan and a doctor’s confirmation that you’ve been excused from work for at least 14 days.
